- Quick Answer: Helaena Targaryen is pregnant in Season 3, likely with Prince Maelor. Because Aegon is severely injured, fans theorize her brother Aemond is the father.
House of the Dragon Season 3, Episode 4, "Tumbleton," hinted that Helaena may be pregnant, and there are theories that the father is not her brother-husband, Aegon, but Aemond.
The HBO prequel has already made several major changes to George R. R. Martin's Fire & Blood. In the book, Helaena has three children and never becomes pregnant again, making this a completely original storyline for the series.
So, if the show is taking this new direction, how could Aemond end up being the father of Helaena's child?
How Could Aemond Become the Father of Helaena’s Child?

Helaena could finally give birth to Maelor in House of the Dragon Season 3 after many assumed the character would be completely removed from the series–a change that even George R. R. Martin criticized.
However, if that twist wasn't significant enough, a popular theory suggests the baby may actually be Aemond's son rather than Aegon's.
While the idea may seem shocking, it wouldn't be out of place in a series built around the Targaryens' incestuous family dynamics. In fact, the show has been dropping subtle hints that could support the theory.
Unlike in Fire & Blood, the series has portrayed Helaena and Aemond as sharing a noticeably closer relationship, giving them a stronger emotional connection than Helaena appears to have with the rest of her family.
Although there's no explicit romantic relationship between them, their bond plays out differently throughout the series. One notable example comes when Helaena reveals that Aemond will die at the God's Eye, a prophecy that visibly affects him and leaves him unusually vulnerable.
By contrast, Helaena's relationship with Aegon has been far more distant. Aemond, meanwhile, consistently treats his sister with a level of care he rarely shows anyone else. He listens to her, takes her visions seriously, and often appears more protective of her than the rest of the family.
The timing of Helaena's apparent pregnancy has also fueled speculation. It seemingly occurs after Aegon flees King's Landing with Lord Larys Strong, leaving Aemond to rule as Prince Regent.
The show has also emphasized Aemond's increasingly distorted view of family and power. In the Season 3 premiere, he even kisses his mother, Alicent, showing his distorted sense of affection and intimacy.
Taken together, these moments have led some viewers to theorize that, now seeing himself as the head of House Targaryen, Aemond could also be the father of Helaena's unborn child.
How This Storyline Would Fit House of the Dragon Season 3?

If Aemond does turn out to be the father of Helaena's child, it would be one of the biggest changes HBO has made to House of the Dragon.
If the baby is indeed Maelor and is a boy, Aemond would become more than just Aegon's capable younger brother–he would also be the man carrying the future of the Targaryen bloodline.
The twist could also give Helaena a much larger role in the series. While her prophetic visions have been important to the story, she has often remained on the sidelines, appearing only briefly in many episodes.
A secret pregnancy would finally give Helaena a major storyline of her own, as she would not only have to conceal the identity of her child's father but also hide the pregnancy itself at a time when Rhaenyra sits on the Iron Throne.
That would place both Helaena and her unborn child in even greater danger and could ultimately lead to the child's tragic fate, much like Maelor's storyline in Fire & Blood.
For now, however, fans should treat this theory with a grain of salt, as nothing is confirmed yet.
