She reluctantly chooses Maelor, but Blood and Cheese kill Jaehaerys instead, leaving the queen consumed by grief and guilt, knowing Maelor is now aware that she chose him to die.
However, the series handles the event very differently. Maelor is completely omitted from the Blood and Cheese sequence and from House of the Dragon Season 2 as a whole.
That may be about to change, though. In House of the Dragon Season 3, Episode 4, Helaena is subtly implied to be pregnant.
Alicent notices the changes in her daughter's body and quickly becomes alarmed, realizing that another heir to the throne could be in danger now that Rhaenyra has claimed the Iron Throne.
It's worth noting that George R.R. Martin never wrote a pregnancy storyline for Helaena in Fire & Blood, as Maelor was already her youngest son in the book.

How House of the Dragon Season 3 Proves George R.R. Martin Right About Maelor

In Fire & Blood, Aegon II and Helaena already have three children when the Dance of the Dragons begins.
After Lucerys is killed, Prince Daemon seeks vengeance with "a son for a son," sending Blood and Cheese to assassinate one of Aegon's heirs. Helaena is forced to choose which of her two sons should die, and believing Jaehaerys, the heir to the Iron Throne, is the more valuable target, she reluctantly chooses Maelor.
By removing Maelor from House of the Dragon, the series significantly changes the Blood and Cheese sequence.
Instead of choosing between her two sons, Helaena just points out who between Jaehaerys and Jaehaera is the boy. While the scene remains devastating, it loses the lasting psychological impact of Maelor knowing that his own mother chose him to die.
That moment is also important because it contributes to Helaena's emotional decline before her eventual suicide, while Maelor later meets a horrific fate when he is torn apart by a mob while escaping King's Landing.
