Did Sister Sage actually kill Deep's octopus and...feed it to him disguised as a blooming onion, before hooking up with him? Did it take a DIY lobotomy to pull it off? We have so many questions about THAT particular scene in The Boys Season 4...
Sister Sage (Susan Heyward), or "just Sage", is a new character introduced in The Boys, and though she has been touted as the smartest person in the world of Vought, she’s been up to some very odd things—such as seducing the Seven’s biggest moron, the Deep (Chace Crawford), over a plate of deep-fried onion.
What’s curious is, at the end of that scene with Deep, there’s a close-up of a bloody needle on Sage’s table, and it’s got a lot of fans thinking about what it could mean.
Sister Sage, The Deep, and the Blooming Onion
To set the scene, the Deep catches Sage alone in her room watching reality TV and eating a blooming onion (an appetizer popularized by Outback Steakhouse in the US). Though Sage hasn’t had any romantic interests so far in the series, she decides to seduce the Deep, much to his enthusiasm.
As Sage made no secret of her disdain for the dimwitted Deep in previous episodes, we can only assume this is part of her sinister endgame.
Sure enough, as they start to hook up, there's an ominous close-up of what looks like a bloody needle on Sage’s table.
Did Sage Kill Deep’s ‘Pet’ Octopus?
A lot of fans have been curious about the needle at the end of the scene, and speculating that it could mean Sage actually killed something that belonged to Deep—specifically his new pet octopus/lover.
For context, Deep has had a thing for octopuses (octopodes?) for a while. After his, uhm, predilection became public in season 3, Deep began hiding his lover/pet in his closet – which is a lot to unpack in its own right.
Earlier in the series, Homelander even forced Deep to eat his pet/lover Timothy as a psychological torture tactic:

What the Sage's Bloody Pick Likely Means
That's one theory. Another is that the needle on Sage’s table is supposed to suggest that she gave herself a lobotomy.
Apparently, being the smartest person on the planet comes with a lot of pressure, and Sage’s way of coping is literally ‘turning off her brain.’ That’s why when Deep catches her in the room, she’s eating junk food, watching reality TV, and suggesting they watch Transformers: Revenge of the Fallen.
While a lobotomy does sound pretty extreme, searching for images of lobotomy tools on Google will show the same kind of pick that’s spotted on Sage’s table.
It’s also worth noting that the show has pointed out that Sage has oderate regeneration abilities, so it’s likely that her dulled state isn’t going to be permanent.
That being said, if Sage didn’t kill the Deep’s pet/lover that doesn’t mean something terrible couldn't still happen to it. The show wouldn’t set up something like a secret pet for The Deep without thinking of some kind of payoff.
Though the obvious choice is that the octopus is going to be discovered and Deep will have to face consequences, it’s possible that The Deep could be forced into turning on Homelander should he be told to eat another one of his pets/lovers.
It’s probably what Timothy would have wanted.
