What happened to Sister Sage in The Boys? One minute, she's the most intelligent person on Earth. Next thing you know, she's making out with The Deep as though there's no residue ripping off the corner of her eye.

Did she really lobotomize herself? What does this imply for her and The Seven?

Sister Sage Lures The Deep with Flavortown

Despite successfully baiting Starlight using Firecracker's antics on live television, Sister Sage is spent navigating her day looking numb and reserved in front of the TV.

The Boys season 4 episode 4 titled A Wisdom For the Ages shows a dark secret that Sister Sage has been keeping from Homelander and The Seven.

When Sister Sage calls on The Deep for help, she's not exactly asking for a companion to eat Flavortown with her to laze around the TV. She tells The Deep that while most people's brains grow until the age of 25, hers regenerates constantly.

Stabbing Sister Sage's brain wouldn't do the trick, either, as she said, it would only grow back moderately, suggesting she had already tried it before.

And so, to compromise with her powers, Sister Sage wanted The Deep to give her a 'frontal lobotomy' right in the corner of her eye.

She would temporarily lose the Supe we have always known her to be for a couple of hours. By this, she means losing IQ points and being free from the burden of her knowledge of the world for a while.

So, technically, The Deep did perform a frontal lobotomy on Sister Sage, as she instructed him to, paired with a surgical instrument meant for 'transorbital lobotomies' called an orbitoclast and a small hammer to 'tap, tap' on her brain.

But now, the question is, did it work? Did the self-lobotomy lower Sister Sage's IQ?

Would Self-Lobotomy Affect Sister Sage's IQ?

The choice of self-lobotomizing her frontal lobe (and the possession of the surgical materials) indicates that Sister Sage has done this before. But what does it do?

The frontal lobe is responsible for a collection of cognitive skills and control over one's behavior and emotions, which suggests that Sister Sage did this after feeling overwhelmed and resigned from her position of lifting The Seven to the public.

Frontal lobotomy is a medical practice that was developed in the 1930s as a way of treating mental illness, particularly depression and schizophrenia. This was considered the most barbaric and brutal medical procedure, except that psychosurgeons used ice-pick hammers.

The 'IQ' affected in Sister Sage upon performing the lobotomy is the control and intelligence of processing which would be the next best move here. In short, this renders Vought and The Seven defenseless and unarmed without Sister Sage's smarts controlling behind the cameras.
