Before becoming a demon lord with Milim and Ramiris, Rimuru was a lowly slime with no eyes, mouth, or nose. Fortunately, Rimuru obtains a human form in TenSura thanks to a fated encounter with a summoned one.

When Does Rimuru Become Human in Tensura?
A lot happened in the first season of TenSura, including Rimuru Tempest's transformation into a humanoid form resembling the magic-born Shizu.
Rimuru began life in the isekai world as a humble slime with no eyes, mouth, limbs, or nose. They were only able to communicate because Veldora made it easier for them to do so face-to-face.
As Rimuru takes on the challenge of leading the Jura Tempest Federation, leading not only goblins and orcs but also wolves, they suddenly obtain a human form following their encounter with Shizu, Kaval, Gido, and Elyun.
At the time, Shizu joined Elyunās party to observe the cave where Veldora was once sealed. They find themselves near Jura Forest, where they eventually encounter Rimuru.
In season 1 episode 8, Inherited Will, Rimuru finally showed their human appearance, which surprised Shizuās friends. They initially thought that Rimuru had killed Shizu, but Rimuru quickly explained what happened to them.
Still, Hinata Sakaguchi of the Western Holy Church sees Rimuru as Shizu's murderer, and she refuses to believe whatever the slime says to her.
Hopefully, Rimuru and Hinata Sakaguchi can discuss this calmly before they fight again.
Previously, Rimuru nearly perished the last time they fought due to Hinata's strength and will to avenge Shizu's death.
How Does Rimuru Become Human in TenSura?
Thanks to Shizu, Rimuru was able to become human in TenSura. Rimuru's appearance would be different if it weren't for her.
When Demon Lord Leon summoned Shizu, he imbued her with the powerful spirit known as Ifrit, which would keep her alive for many years.
So, when Rimuru consumed Ifrit during their battle with Shizu, the latter didn't have much longer to survive.
On her deathbed, Shizu asked Rimuru to use their predatory skills on her body as her final wish.
Rimuru acknowledged Shizu's request in the hopes of fulfilling her ambitions, which include finding her master as well as saving her otherworldly students.
With this, Rimuru can turn themselves into a humanoid form that looks like Shizu.
They can also alter their body at will, whether they want a youthful or more adult look, depending on their preference. They can also switch genders, but Rimuru is not comfortable with it.
Despite having a human body, Rimuru can still switch back to his slime form and has had no difficulty doing so.
