In the end, Wanda, herself, knew that the Darkhold has consumed her and to make sure that no one else would be able to access the Darkhold ever again, Wanda brings down the whole temple of Chthon, where the original spells of the book of the Damned are written on the walls.

Wanda went down with the temple but there's a hint that she may still be alive given thag after it crumbled down the mountain, Wanda was able to destroy every version of the Darkhold in the whole multiverse as the version beside Strange gets destroyed by Wanda.

America Chavez now trains with the sorcerers at Kamar Taj and she thanks Strange for being different and believing in her. Back at the Sanctum Sanctorum, Strange repairs his watch and while he casually walks outside, he fell on his knees in the middle of the street, in pain. A third eye comes out of his forehead.

Notable points in the end of the film are first, Wanda may or may not have survived the destruction of the temple of Chthon but given her powers, she may be very well alive as the darkness in her fades away along with the Darkhold's destruction.

Lastly, Doctor Strange having a third eye only means one thing: he now has the Eye of Agamotto. The Eye of Agamotto in the comics is a magical talisman that was made by none other than Agamotto himself, one of the three mystical god-like beings known as the Vishanti.

It is held by the Sorcerer Supreme, and Agamotto is the first to hold that title. The Eye sees and shows the truth and it casts an all revealing light which shows what happened to get to the real truth. It cannot be used for evil, the very reason why Doctor Strange finally had one, he reached the peak of being the supreme version of himself.
