Why Hae-In Refused To Undergo Surgery in Queen of Tears
Hong Hae-In (played by Kim Ji-Won) had been reluctant to take on her husband, Baek Hyun-Woo (Kim Soo-Hyun), advice to undergo surgery, for a reason that could pretty much affect the development of their relationship moving forward: memory loss.
Considering that Hae-In's life is hanging in the balance in Queen of Tears, she has two options: push through surgery and lose her memories or avoid surgery altogether and lose her life.
One outweighs the other, and Hae-In would rather die than lose who she is and her memories with Hyun-Woo.
However, a pinnacle moment leads Hae-In to choose to live with memory loss, and that is when she witnessed how hurt Hyun-Woo had been when he initially thought he lost her in a car accident.
Out of empathy, especially after seeing how much Hyun-Woo doesn't want to lose her, Hae-In made the difficult decision to opt for surgery. But things don't exactly go the way they were planned.

What Happened to Hae-In's Diary? Did Hyun-Woo Lose It?
Right before Hae-In's surgery in Queen of Tears episode 14, she hands over her little black diary to Hyun-Woo, as a reminder of who she was before she lost her memories after the operation and for Hyun-Woo to hold evidence of every memory they had together.
However, while his wife was detained in the operating room, Hyun-Woo was suddenly under arrest after being framed for murder. This leaves fans with the lingering question, what happened to Hae-In's diary?
Well, Hyun-Woo was last seen holding the diary in his hands right before he was arrested for framed murder. Despite his pleading, nobody believed he wasn't the culprit.
And so, once he was forced out of the picture, a "guardian" came in his place and took over.
As for the diary, though, Hyun-Woo left it in the chair he was sitting on while waiting for news on Hae-In's condition.
Nobody's claimed it yet as of episode 14, but Hyun-Woo should definitely remember to pick it up if he wants to reunite with Hae-In.
