As it turns out, Lieutenant Kato (played by Choi Young Joon) is hellbent on experimenting with turning people into monsters with a parasite called Najin.

In the opening credits of the series, we learn that the parasite came from the ocean, which the Japanese encountered after a girl destroyed her village.

Lieutenant Kato is enticed by the idea of such a creature, which he refers to as a "goddess." With his determination, he experiments on the different Joseonjing prisoners who have gone missing.

Out of all the women he experimented with, only Choi Seong Sim (played by Kang Mal Geum) survives and turns into the monster in Gyeongseong Creature.

In the first episode of Gyeongseong Creature, Commissioner Ishikawa (played by Kim Do Hyun) picks out two women to become the new test subjects. One of the women he picks is Seong Sim, who is the mother of Chae Ok.

Seong Sim has gone missing for a decade, which has led Chae Ok and her father to become sleuths. They are determined to find Seong Sim, which leads them to Master Jang's aid.

After getting picked, Seong Sim and the other woman are separated into two cells. A plate full of rice cakes and a glass of water were given to them. What they didn't know, however, is that the water contained the Najin.

This leads the two women to get sick and become mad. The older of the two women, Seong Sim, exhibits less severe effects. Meanwhile, the younger woman starts to crush her head on the floor.

Seong Sim thus becomes Lieutenant Kato's first successful test subject, aka the monster in Gyeongseong Creature.
