What was Death's Promise to Orpheus?
If there's one thing Death doesn't do, it's intentionally killing people off the pages and episodes of The Sandman. So, imagine the fans' surprise when Death "broke" her promise to Dream and Orpheus. But what exactly was that promise?
Death essentially granted Orpheus immortality, as per Dream's wish. However, this little 'intervention', as she warned Orpheus, was not meant to be seen as a gift, but more akin to a curse she won't foresee on her death list.

This promise came right in time when Orpheus sought out his aunt, Death, for help upon the death of his newly married wife. Death was able to send Orpheus down to the Underworld in his attempt to retrieve his wife.
However, as the classic mythology went, Orpheus broke a vow of his own and lost everything he worked for.
Did Orpheus Die in The Sandman Season 2?
Hades and Persephone had only one condition for Orpheus to have his wife return to his side, and that was to never look back as he walked towards the light.
Despite that, Orpheus couldn't resist turning his head to make sure they had kept their end of the bargain.
Thanks to that mistake, his wife got pulled back, and Orpheus was disembodied by the Sisters of the Frenzy, a cult devoted to Dionysus, leaving him with a decapitated head. And honestly, no one wants to live life in such a state.
This led Orpheus to seek one more wish from his father: to kill him and rid him of his immortality. Like any other father, Dream hoped to give his son his final peace, and if that was the afterlife, then so be it.

Dream could commit his son's death wish, but it would mean two things: asking a boon in exchange for Orpheus to be taken to the Underworld, and risking the consequences of facing the Furies' wrath for committing filicide.
Both of which Dream achieved by the end of episode 6, titled Family Blood, as he killed his son with his bare hands, awaiting the potential threat looming behind his back.
